The bomber set off a vest packed with explosives in al-Abbara, near the restive city of Baquba, police say.
Al-Qaeda militants have been fighting government forces and local tribal groups in the province.
News agency reports quoting police and medical officials say the blast occurred in a village coffee shop.
The attack came after a suicide car bomber struck a police checkpoint in central Baquba, killing one policeman and a civilian, police say.
Six people were wounded in that attack, three of them children and three, according to one report, policemen.
Diyala province also saw attacks on Monday in which at least four people were killed.
Also on Tuesday, a parked car bomb aimed at a police patrol in Baghdad went off, killing four people and wounding seven others.
